What Challenge Will You Embrace?
As a Trajectory Guide at Movens, you will provide personalized guidance to clients, empowering them in selecting, acquiring, and maintaining meaningful day activities, volunteer work, or paid employment, as well as education or training. Your role is to assist them in discovering suitable, enjoyable, and purposeful day engagement.
You will achieve this by:
Conducting intake interviews with new clients to explore suitable day activities and motivate their participation. This includes working with clients living independently as well as those residing in protected housing within the Duin en Bollenstreek.
Collaborating with the client and/or relevant stakeholders to develop action plans, set clear objectives, and communicate these with the activity coordinators on-site.
Evaluating progress biannually, engaging clients in discussions about their day activities, and jointly assessing if adjustments are necessary.
Working intensively with other trajectory guides, the coordinator, and activity facilitators at Movens, participating in case discussions and regular meetings.
Monitoring the process surrounding re-assessment and ensuring that WMO decisions for day activities are timely renewed.
