Qualifications
Essential Responsibilities:Develop individual community support plans with specific goals (typical caseload: 40-45 individuals).Assist individuals in accessing services and create service agreements, including thorough case documentation.Educate individuals or their legal guardians about appropriate service options.Conduct biannual one-on-one meetings with each individual to assess progress and adjust care plans as necessary.Identify potential service providers to help individuals achieve their goals.Evaluate and monitor the effectiveness of services outlined in the care plan.Ensure compliance with case management rule 185.Adhere to Minnesota Health Care Programs related to waiver services, including completing MnChoices assessments and utilizing required state systems.Requirements:Bachelor's degree in social work, nursing, psychology, sociology, or a closely related field from an accredited institution; or a Bachelor's degree in another field with one year of relevant experience in social services as a supervised social worker.If you possess a social work degree, licensure by the Minnesota Board of Social Work is required.Strong written and verbal communication skills.Ability to work independently and handle crisis situations effectively.Comfortable managing a high volume of paperwork.Detail-oriented and process-driven.Must pass a background check by the Department of Health Services, possess a valid driver's license, and maintain a clean driving record.Reliable transportation is necessary.
