Job Overview:
As the Clinical Lead, you will serve as the primary clinical owner of each assigned case, ensuring that all aspects of the care plan are designed, coordinated, and executed with the utmost clinical integrity. Responsibilities include conducting assessments, formulating treatment plans, engaging with caregivers, and fostering interdisciplinary collaboration.
You will act as a vital link between clinicians, Developmental Paraprofessionals (DPPs), and families, ensuring adherence to the Positive Development model through strategic case planning, effective communication among care team members, and diligent outcome tracking.
Key Responsibilities:
Facilitate initial meetings with parents to collect family history, objectives, and concerns
Conduct in-person clinical assessments and document developmental requirements
Create and uphold treatment plans aligned with Positive Development’s care model and family priorities
Lead regular family support sessions (both virtual and in-person), offering guidance and education
Ensure effective communication within the care team and keep all providers (OT, ST, MHC, DPP) informed and coordinated
Monitor progress towards goals and adjust treatment plans as necessary
Prepare and submit documentation for initial and ongoing reauthorizations, including updates to treatment plans and outcome metrics
Provide formal clinical supervision
Participate in internal case reviews and work collaboratively with Clinical Subject Matter Experts (SMEs) to ensure quality and fidelity
Identify clinical risks and escalate issues as appropriate
Assist the DPP Manager and align direct service with clinical objectives
Home and community visits may be required
Qualifications:
Master’s degree and an active clinical license (LCSW, LMFT, LPCC) required
Minimum of 3 years of experience in developmental, behavioral, or autism-related services
Proven ability to lead clinical planning, coordinate interdisciplinary care, and support caregivers effectively
Excellent written and verbal communication skills, with a strong focus on documentation quality and clinical compliance
Demonstrated experience in case management and clinical supervision
