Join Guidestar Eldercare as a Nurse Practitioner and make a meaningful impact on the lives of the elderly facing dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. This role offers the unique experience of private practice autonomy, coupled with a designated caseload, collaborative corporate clinical leadership, and comprehensive administrative support from our in-house scheduling, coding, and billing teams.
As a vital member of our multidisciplinary team focused on enhancing patient safety, functionality, and dignity, your role will involve:
Traveling daily to assigned facilities from your home base.
Conducting initial evaluations, providing ongoing therapy services, and performing cognitive assessments.
Documenting the needs and extent of services for each patient.
Building and maintaining positive professional relationships with client facilities, patients, and their families.
