Position Overview:

As we continue to expand our commitment to delivering top-quality care to our members, Advantmed is in search of a dedicated and skilled certified Nurse Practitioner to enhance our provider network. This role presents an exciting opportunity for those looking to make a significant impact on patient care.

 

As a Nurse Practitioner, you will be entrusted with conducting in-home wellness risk adjustment assessments for Medicare beneficiaries and other vulnerable populations. Your primary responsibility will be to evaluate the overall health and well-being of our members, ensuring precise and thorough risk adjustment coding that facilitates superior value-based care. This position operates on a 1099 (PRN) basis, offering flexible scheduling to accommodate various personal commitments.

Primary Responsibilities:

Conduct in-home clinical assessments including:

  • Traveling to members' residences to perform comprehensive health evaluations.
  • Executing Annual Wellness Visits and health assessments for members with chronic diseases.
  • Performing essential medical evaluations such as:
    • Thorough medical history and physical examinations.
    • Systems review.
    • Collection of vital signs.
    • Medication review and reconciliation.
    • Conducting preventive health screenings, including Hemoglobin A1C, Spirometry, KED (Kidney Health Evaluation), DRE (Diabetic Retinal Exam), and FIT.

Patient Education & Engagement:

Delivering personalized health education to empower members in managing their health effectively.
