Your Role
As a member of the Porter team, you will be a catalyst for change, redefining healthcare delivery through empathy and innovation. Your excitement and energy will contribute to our mission of enhancing the healthcare continuum for our members.
Key Responsibilities
You will conduct in-home health risk assessments for our members, evaluating their medical history, reviewing systems, reconciling medications, and performing comprehensive physical examinations. This role emphasizes evidence-based practice, including point-of-care testing and diagnosis identification for effective care management in collaboration with our Porter Care Guides.
Qualifications
- A valid, unrestricted Michigan Nurse Practitioner license
- Minimum of 3 years of patient care experience, preferably in primary care, adult/geriatric settings, with EMR proficiency
- Strong empathy, compassion, and approachability
- Outstanding verbal and written communication skills, with a commitment to supporting patients and colleagues with respect and kindness
