Your Role

Key Responsibilities

You will conduct in-home health risk assessments for our members, evaluating their medical history, reviewing systems, reconciling medications, and performing comprehensive physical examinations. This role emphasizes evidence-based practice, including point-of-care testing and diagnosis identification for effective care management in collaboration with our Porter Care Guides.

Qualifications

- A valid, unrestricted Michigan Nurse Practitioner license
- Minimum of 3 years of patient care experience, preferably in primary care, adult/geriatric settings, with EMR proficiency
- Strong empathy, compassion, and approachability
- Outstanding verbal and written communication skills, with a commitment to supporting patients and colleagues with respect and kindness
