What You’ll Need: Board Certified with a valid Nurse Practitioner (NP) or Physician Assistant (PA) certification in the State of New York. Strong clinical skills with the ability to assess, diagnose, and treat confidently. Excellent interpersonal skills and a positive attitude. Ability to work independently and solve problems effectively.1-2 years of experience in primary care, preferably with geriatric patients (hospice experience is a plus, but not mandatory).

Purpose: 

We are seeking a skilled Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients within the Gravesend/Flatlands, NY region. You will apply your clinical expertise under the guidance of a physician, caring for a diverse patient population with chronic illnesses.

Responsibilities: 

  • Deliver high-quality care to primary and palliative patients in the Gravesend/Flatlands area.
  • Conduct comprehensive health assessments through physical examinations, and review medical histories to formulate optimal care plans.
  • Communicate effectively with office staff and clinical teams via various internal platforms.
  • Provide patient-centered care to a dedicated panel of patients.
  • Participate in quarterly medical staff meetings.
  • Develop diagnostic impressions, treatment plans, and follow up on patient care.
  • Build trust and consult with patients, families, and caregivers during home visits.
  • Order necessary diagnostic tests as part of patient care.
  • Prescribe medications and medically necessary interventions.
