Role overview

This full-time position is for a Nurse Practitioner or Physician Assistant based in Montgomery County, MD. The clinician will deliver primary and palliative care to homebound patients, working under physician supervision. Many patients have chronic illnesses and require ongoing, patient-centered support.

What you will do

  • Provide primary and palliative care to patients in the Montgomery County area.
  • Conduct comprehensive health assessments, including physical exams and medical history reviews, to inform care plans.
  • Communicate with office staff and clinical teams across multiple platforms.
  • Manage a dedicated panel of patients, focusing on individualized care.
  • Develop accurate diagnoses and treatment plans.
  • Collaborate with patients, families, and caregivers to foster trust during in-home visits.
  • Order diagnostic tests as appropriate.
  • Prescribe medications and implement interventions when medically indicated.

Requirements

  • Current Nurse Practitioner or Physician Assistant license and board certification in Maryland.
  • Strong clinical judgment with the ability to assess, diagnose, and treat patients independently.
  • Excellent interpersonal skills and a collaborative approach.
  • Ability to solve problems independently.
  • 1-2 years of primary care experience, ideally with geriatric patients. Hospice experience is helpful but not required.
