Our dedicated team is focused on nurturing patients' health and well-being directly in their homes.Compensation: Enjoy competitive pay with a structure of $64.10 per point, translating to an annual salary range of $85,000 to $105,000.Mileage Reimbursement: Additional compensation for mileage on top of your productivity earnings.Schedule: Full-time position with a Monday to Friday schedule, along with some weekends as needed.Work Environment: This position is field-based, allowing you to make a meaningful impact in the community.About Us:At Adaptive Home Health, we are revolutionizing the home health care landscape across Texas. Our innovative charting and scheduling products aim to streamline processes, reduce administrative burdens, and enhance visit efficiency.If you possess strong clinical judgment and are committed to driving patient outcomes, this role offers autonomy, a significant patient impact, and a supportive team environment designed to minimize administrative challenges.Your Responsibilities:Conduct thorough patient assessments and manage skilled visit documentation, including OASIS.Provide high-acuity home health care, specializing in wound care and infusion therapy.Develop and uphold a clear plan of care through consistent communication with physicians, care team members, patients, and their families.Act as an advocate for patients, ensuring continuity of care across various disciplines.Manage case responsibilities, including care coordination, timely follow-ups, and resolving patient barriers to progress.Communicate any changes in patient conditions, track medical orders, and coordinate services like PT/OT/ST/MSW/HHA to ensure effective care delivery.Administer prescribed medications and treatments while providing education to patients and caregivers.
