Job OverviewWe are seeking a dedicated Field Nurse Case Manager to serve as a crucial liaison in coordinating care for injured workers. In this role, you will effectively communicate essential medical information among the Injured Worker, Insured, and Carrier to ensure optimal treatment, rehabilitation, and a successful return to work. You will be responsible for providing efficient and cost-effective communication concerning workplace injuries, motor vehicle accidents (MVAs), liability claims, long-term disability (LTD), short-term disability (STD), and additional services as needed. This position is primarily remote and requires regular local travel of up to 2 hours within your designated region.Key ResponsibilitiesAdhere to all policies and procedures outlined in the Case Management (CM) plan, ensuring accountability throughout the CM process.Accept referrals as assigned by the Director of Case Management.Initiate contact according to the CM plan, schedule initial assessments, and determine the date of the next physician appointment.Confirm assignments with referral sources and clarify any special handling instructions.Develop professional relationships with clients while treating claimants with respect and dignity.Post-physician appointments, follow up with the Carrier and Insured according to protocols, ensuring consistent communication with the Injured Worker, Insured, and Carrier.Document data and billing within CaseAnyplace and submit timely monthly reports (Preliminary, Initial, Progress, Closing).Attend physician appointments to gather diagnosis, prognosis, treatment plans, rehabilitation durations, return-to-work estimates, and maximum medical improvement (MMI) or full recovery information as necessary.Refer to a Vocational Counselor for job analyses when appropriate and as approved by the Carrier.Recommend Independent Medical Exam (IME) physicians and coordinate attendance at IMEs.Facilitate transportation as needed.Provide translation services if applicable (for bilingual nurses).Monitor treatment plans, attend therapy sessions as appropriate, and maintain communication with therapists for updates.Request the transfer of files to Vocational Counselors when required (LMS, Voc Rehab).Assist the Carrier/Insured with return-to-work planning (modified or regular duty).Supply information to Defense Attorneys as needed.Encourage teamwork and collaboration among all staff members.Maximize accurate and appropriate billable hours to meet monthly targets (8 hours/day).
