Dementia Case Manager in Mijdrecht
24 – 32 hours per week
Support individuals with dementia and their loved ones as a compassionate guide, coordinator, and listener.

As a Dementia Case Manager, you provide clarity, trust, and tailored support. You will work independently and collaboratively within a passionate team in a dynamic organization.
Your Role:
As a case manager, you will guide clients with dementia through their care journey. The increasing aging population and the trend of aging in place lead to more complex care needs for these individuals. As an independent advisor, you will monitor the needs and preferences of clients and their families. You will act as the link between the client and various services, ensuring a smooth care process. Collaboratively seeking the best solutions for diverse challenges is what makes this role rewarding. You will build relationships, assist, and support the client. Your knowledge of the intricate laws and regulations will be well-maintained, as changes are frequent and we focus heavily on this within the organization. Engaging with other disciplines, such as general practitioners, welfare organizations, municipalities, and CIZ, adds an exciting dimension to this challenging role. It is crucial to align tasks and responsibilities effectively.
A Day in the Life of a Case Manager at Zonnehuisgroep Amstelland
No two days are the same in your role as a case manager. You start your day with a cup of coffee or tea while reviewing your messages. A concerned son reports that his mother is forgetting her medication. You call him, offer advice, and coordinate with district nursing or the digital team if necessary.
Next, you visit a client with a recent dementia diagnosis. You discuss what this diagnosis means, suggest day activities, and identify caregiver burnout – immediately applying for WMO support.
