Join our team as a Health Program Representative in Houston, TX, where you will serve as the primary liaison between various Care Management programs and designated members. In this role, you will manage inbound and outbound enrollment and engagement calls, providing essential information about program features and benefits.
Key Responsibilities:
- Collect and document vital member information in compliance with established policies and procedures during phone interactions.
- Motivate members to engage with Care Management programs by effectively communicating the program's features, values, and available services.
- Accurately log call details and utilize call scripts in alignment with client specifications; direct member inquiries to the appropriate clinical personnel.
- Facilitate referrals and manage consultations from clinicians for registered participants in Care Management.
- Gather information from hospitals, health plans, physicians, patients, vendors, and other referral sources as required.
- Contact physician offices to obtain necessary demographic information or related data.
