Qualifications:Minimum of two (2) years of full-time experience or part-time equivalent in a mental health setting. A bachelor’s degree in psychology, social work, or behavioral sciences may substitute for the two (2) years of full-time work experience or its part-time equivalent. Proven knowledge of DSM-5 and principles of psychosocial rehabilitation and recovery. Familiarity with regulatory standards in the mental health field.

Job Title: Service Coordinator (Mental Health Case Manager)

Key Responsibilities: As a Service Coordinator (Mental Health Case Manager), you will assess clients to pinpoint individual challenges requiring treatment and conduct comprehensive evaluations. You will also facilitate interdisciplinary team meetings and oversee discharge planning alongside a multidisciplinary treatment team. In this role, you will manage the implementation of the client's overall treatment plan, collaborating closely with nursing staff to ensure consistent treatment efforts.
